Your privacy is important to us.

As a psychology practice, we collect and hold personal information that is necessary for us to provide psychological services. This Privacy Policy explains how Freedive Psychology collects, holds, uses and discloses personal information and how individuals may access or make a privacy-related complaint.

 

What personal data we collect and why we collect it

We collect certain personal data to provide you with the products or services you request, and to meet our professional, ethical, regulatory and legal obligations. This personal data may include:

  • Personal details including name, date of birth, email, mobile telephone number and address

  • Emergency contact details

  • Parent, guardian or carer details for anyone under the age of 18

  • Medicare, or other government-related information where required for the provision and/or funding of services

  • Referral information

  • Clinical notes and treatment records

  • Psychological assessment results, questionnaires and psychometric data

  • Psychological reports and correspondence

  • Information provided by GPs, psychiatrists, paediatricians, schools, allied health professionals and other relevant third parties

  • Appointments and attendance information

  • Payment or credit card information

  • Information provided through our website enquiry form, intake forms, email, telephone or other communications\

 

​How we store personal information

 

Freedive Psychology takes reasonable steps to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. Our primary practice management system is Halaxy, which is used to manage client information including clinical records, referrals, appointments, intake information, invoicing, payment information and Medicare information. Personal information may also be stored or processed through systems used by the practice for email and communication, AI note taking, psychological assessment, telehealth, website administration, accounting and other necessary practice functions. Access to clinical information is limited to practitioners and other authorised personnel who require access for legitimate clinical or administrative purposes.

 

Disclosure of Personal Information

 

Personal information is not ordinarily disclosed to another person or organisation without consent unless the disclosure is permitted or required by law, including in circumstances involving a serious threat to an individual's life, health or safety or to public health or safety, or where disclosure is required by a court, tribunal or other lawful authority. ​Our clinicians also engage in regular clinical supervision to support safe and effective care. Relevant aspects of your care may be discussed in supervision, with information de-identified where practicable and confidentiality maintained. 

 

Access to Personal Information

Individuals may request access to personal information that Freedive Psychology holds about them. Requests should be made in writing to Freedive Psychology. We will respond to requests within a reasonable period. Access may be provided in different forms depending upon the circumstances. There may be circumstances in which access may be refused or limited where permitted by law. Where this occurs, we will generally provide written reasons for the decision where required.

Retention and Disposal of Records

 

Freedive Psychology retains psychological and clinical records in accordance with applicable legal, professional and regulatory requirements. When personal information is no longer required to be retained, Freedive Psychology will take reasonable steps to securely destroy or de-identify the information where permitted and appropriate.

 

Privacy Complaints

If an individual has concerns about how Freedive Psychology has collected, used, stored or disclosed their personal information, they are encouraged to contact us. Privacy complaints should be made in writing and should provide sufficient information for us to understand and investigate the concern. Freedive Psychology will acknowledge and investigate privacy complaints and seek to respond within a reasonable period. If an individual is dissatisfied with our response, they may be entitled to lodge a complaint with the Office of the Australian Information Commissioner (OAIC) or another appropriate regulatory body.

 

Data Breaches

Freedive Psychology maintains processes for responding to suspected or actual privacy and data-security incidents.

Where a data breach occurs, we will assess and respond to the incident in accordance with applicable legal requirements, including the Notifiable Data Breaches scheme where applicable.

 

Changes to This Privacy Policy

Freedive Psychology may update this Privacy Policy from time to time to reflect changes to our services, technology, legal obligations or information-handling practices. The current version will be made available through the Freedive Psychology website.
